IMPLEMENTASI PROGRAM INOVASI REKAT (RENCANA EDUKASI KESEHATAN PASCA RAWAT) SMART DISCHARGE PLANNING BERBASIS EDUKASI ELEKTRONIK DI RUANG RAWAT INAP TIU KELEP RSUD PROVINSI NUSA TENGGARA BARAT

Discharge planning yang kurang optimal pada pasien rawat inap dapat meningkatkan risiko readmission dan menurunkan mutu pelayanan keperawatan. Penelitian ini bertujuan mengevaluasi implementasi program REKAT (Rencana Edukasi Kesehatan Pasca Rawat) sebagai inovasi edukasi elektronik berbasis kode respons cepat (QR code) untuk meningkatkan kesiapan pasien dan keluarga dalam perawatan pasca pulang. Studi ini menggunakan desain kuasi-eksperimen pre–post test tanpa kelompok kontrol pada 64 pasien dan keluarga di Ruang Rawat Inap Tiu Kelep RSUD Provinsi Nusa Tenggara Barat. Intervensi berupa pemberian edukasi discharge minimal satu hari sebelum pulang melalui leaflet elektronik dan metode teach-back. Hasil menunjukkan peningkatan signifikan skor total pemahaman, sikap, dan kesiapan pasien (+1,05 poin), dengan peningkatan terbesar pada pengetahuan obat (+1,2 poin) dan tanda bahaya (+1,1 poin). Kepatuhan perawat terhadap prosedur mencapai 85%, kepuasan pasien 90%, serta penggunaan leaflet cetak menurun 60%. Temuan ini membuktikan bahwa REKAT efektif meningkatkan kesiapan pasien sekaligus efisiensi sumber daya, sehingga layak diintegrasikan ke dalam sistem informasi rumah sakit.
