Abstract

Background: Low Birth Weight (LBW) infants, especially those born prematurely, are highly susceptible to respiratory system disorders. One of the common problems is an ineffective breathing pattern, caused by immature lungs that are not yet optimal in producing surfactant and underdeveloped respiratory muscles.
Method: This research is a qualitative case study aimed at describing in depth the implementation of nesting therapy to address ineffective breathing patterns in LBW infants. The subject of the study was one LBW infant patient treated at Baladhika Husada Hospital (DKT) Jember. The inclusion criteria included infants with a birth weight of less than 2500 grams. The infant had a birth weight of 2200 grams and a body length of 39 cm, with a weak sucking reflex observed 4 hours prior to hospital admission. Results: At the initial assessment, the infant presented a respiratory rate of 37 breaths/minute, SpO₂ of 92%, no chest wall retraction or additional breath sounds, and mild cyanosis on the lips. On the first and second day, the infant required supplemental oxygen. However, on the third day, the infant showed improvement and no longer needed oxygen therapy. The respiratory rate remained within normal limits and there were no signs of accessory muscle use. On the fourth day, the infant’s condition stabilized with a respiratory rate of 40 breaths/minute and SpO₂ increased to 99%. Conclusion: The implementation of nesting therapy was effective in stabilizing the breathing pattern of an LBW infant. This intervention can serve as a non-pharmacological approach that supports respiratory recovery in premature infants with ineffective breathing patterns.
